Security (or lack of?)
BOP used to have a rule of one GS-9/GS-11 LT and four CO's on each inmate when taken outside the confines of the facility for med appts or hospital trips. Always two vehicles (prisoner in one and a chase or lead car). I'm not going to second-guess why there were only two armed CO's on what appears to be a high-risk inmate, but it makes me wonder.
Unrelated to the above, but on a related issue to the topic: Don't remember the agency, but we released one inmate to deputies from a Mid-West SO. They drove to LAX for the flight back home. Inmate says he has to pee, one Deputy escorts said inmate to the men's room, and as he's removing one cuff to re-cuff to the handi-cap rail in the stall, the inmate bucks and takes Deputy's gun and disappears. Good news from this is he was captured a few days later.
