You might just have to do the legwork on that one, and come up with your own. Since it is unique to your area, you want to make sure it is worded specifically for what you want to charge them with.

There should be some kind of court recognized UTC or minor misdemeanor cite for your state that all violations go on.

Don't know what the law is in your state but where I'm at, all citation forms must first be approved by the state Judicial Council before they can be put into use.

I'm not a cop; I don't even play one on TV, but would any type of citation have to go through myriad government channels to be legally enforceable?
Embarrassingly, I earned one for jaywalking many years ago. It was the same green souvenir given for driving infractions, of which I also earned a few in my younger, more bullet-proof days.

It'd help if you told us what your community/jurisdiction is and what kind of agency you are.
Here we just use our "Notice to Appear" for pretty much everything. When we had contract cities, we had separate city/county ordinance citations, but those went away.
