Filling out a personal history booklet. It asks for job performance evaluations for each job I have had. Any idea what they are asking for? I've only worked one job where there was a paperwork evaluation filled out. Obviously Ill contact them on Monday when office is open again but thought you may have insight in the meantime.

If you've only worked one job where these evaluations were done, your task should be pretty simple. What they are asking for is the written, job performance evaluations, done on you by a Supervisor, while you worked for the company in question.
