  • Feb '06 : Birth Cert ?

    Hi guys!
    I know that it was posted in another thread somewhere which I've tried to locate, but how do I go about obtaining an original copy of my birth certificate and in average how long does it take to be sent out or can I pick it up? Got my first phone call from my BI yesterday and have 2 weeks to get these documents to him. Thanks so much for the help and good luck to everyone!

    Here is the link to the Cook County Clerk Office. You can request your birth certificate (Certificate of Birth) by filling out the request form, and either mailing it in, or going in person to one of the clerk's offices.
    (I recommend going in person since it takes 3-4 weeks if you mail it).

    All of the information is available on this website.


    Do you have a list of the documents that the BI is going to ask for? This might help to expedite the process for other candidates (like me).

      Some just off of the top of my head are:

      Originals:
      Birth Certificate (if you have kids, theirs too)
      Official driving abstract
      Copies of:
      Driver's License
      Marriage License (if applicable)
      3 Bills showing your address
      FOID Card
      School Loan Info (if applicable)
      Filing of Bankruptcy (if applicable)
      If you have applied to other police agencies
      Selective Service
      Mortgage Bill or Renter's receipt
      Check Stub
      Contact info for 3 of your neighbors
      vehicle registration receipts
      city sticker receipts

        Here is the exact list he gave me of the documents that I need to obtain. I wonder why I was requested to obtain so little vs. others???

        Originals:
        Birth Certificate

        Copies:
        Drivers license
        Vehicle registration
        Current vehicle insurance card
        FOID card


        2 pieces of mail as proof of my residency
        3 names and telephone numbers of neighbors

        I have two weeks to submit these

          Every BI is different..I got mailed a list of over 20 things...only 12 maybe pertained to me. I had to get a copy of my driving record, a freind of mine didn't...

            Thanks for the lists. It looks similar to the list that the county asked for after the PAT, when they have you come in for an initial 'interview'.
            I have my marriage and birth certificates from when I applied for a passport, and the rest shouldn't be too hard to get.

              So then do some BI's mail the applicant vs. phone them? Do you think that after I obtain these documents that he will request more? He said as soon as I have these that we will then set up an interview.

                Goldylocks. I actually got a couple of phone calls from my BI. First was more an intro, then a follow up with a few questions. The final call I received from him was more of a "get the following items" before he came to my house. I dont have much since I dont own anything and dont pay rent (live at home with parents). So basically he just wanted my Birth Certificate, and copies of my FOID, DL, and a list of my neighbors to interview and my manager's work number.
                I was told I was one of the easiest ones to do a background investigation on.

                  I'm in the same boat as you. I'm 23 years old, am still at home with the p's, don't own anything and was only required to gather a few documents. Have you heard back from your BI after your in home interview? Have you gotten a start date yet? When was your psych, medical and in home and do you know your lottery #?
