I doubt he'll be permanently "blackballed", but i'm sure he'll be disqualified until he's able to start the testing process over again.

Its a professional courtesy to at least notify the dept. where you missed the physical agility test. That will at least show that you didn't just blow it off!!

I missed mine for Metro transit but called the morning of while stuck in traffic...They have no problem rescheduling for me
